What types of conditions do you treat?

We treat head to toe pains such as migraines, neck, back and joint pains. We also treat conditions such as post-surgical pain, compression fractures, cancer pain, fibromyalgia, and sciatic pain.

What procedures do you do?

We perform joint injections (sacroiliac or peripheral), nerve blocks (both sympathetic and peripheral), epidural steroid injections, medial branch blocks, facet joint injections, discograms, kyphoplasty, spinal cord simulation, intrathecal pump evaluation, botox, SPG block and outpatient medication detox.

What injections do you perform?

We perform nerve blocks, steroid injections, and several joint injections, also including Botox.

What insurances do you take?

We accept most major insurances. Please call our office to see if we are in-network with your insurance.

What are your hours?

We are open Monday-Friday 8am-5pm. We do not close for lunch.

How soon can I be seen?

We can typically schedule first time consultations within two weeks.

Do I need a referral/authorization to make an appointment?

Yes, most insurances require a referral or prior authorization for each visit. Authorization are also required to treat workers compensation patients.

What are your office Policies?

Our office DOES NOT allow early refills. You are provided with a 30 day supply of medication. Over taking medication is a violation of our office policy & controlled substance agreement contract, which will conclude to being discharged from our office.

We also collect urine samples at RANDOM. When asked for a urine sample, it needs to be provided at the time of your visit in order to obtain your prescription.
